We are seeking a dedicated and compassionate Case Manager – Adult Services to join our team. This pivotal role offers a competitive starting pay of $22.32 per hour, with potential increases up to $23.18 based on experience and qualifications. What You'll Do:

As a Case Manager, you will be at the forefront of coordinating services and supports for adults within a highly regulated environment. Your role involves building meaningful relationships with clients, families, and community providers, ensuring person-centered planning, and monitoring services to promote health, safety, and autonomy. You'll conduct in-person visits, navigate community resources, and utilize innovative systems like The Vault for streamlined access and documentation. Training is structured to support your success, beginning with intensive onboarding and shadowing, transitioning into remote and field work.

Required Skills:

  • Strong understanding of person-centered case management principles
  • Excellent communication and interpersonal skills
  • Ability to adapt quickly to changing rules and regulations
  • Proficiency in computer technology including Microsoft 365 (Teams, Word, Excel), Outlook, ZOOM, and familiar with data management systems
  • Valid Colorado driver’s license and reliable vehicle with appropriate insurance
  • Highly organized with excellent attention to detail
  • Compassionate with a problem-solving mindset

Nice to Have Skills:

  • Experience with Medicaid Waiver programs and community resource development
  • Working knowledge of systems like The Vault
  • Prior experience working with persons with disabilities or in long-term services and supports (LTSS)

Preferred Education and Experience:

  • Bachelor’s degree in human behavioral sciences, social work, psychology, or a related field
  • Relevant experience working with the LTSS population, whether through employment or volunteer work

Other Requirements:

  • Ability to perform community visits, including lifting up to 20 pounds
  • Willingness to participate in ongoing trainings and team meetings
  • Flexibility to work in various environments: office, community, private residences, and virtual settings
