Your Impact:

Your Daily Activities:
- Customer Service Excellence: Utilize effective communication to deliver outstanding service.
- Strong Communication Skills: Exhibit excellent written and interpersonal communication abilities.
- Regulatory Adherence: Understand and comply with LTC and IHSS regulations.
- Data Analysis: Review case documentation, medical and behavior reports, financial records, and more to support clients’ health and well-being.
- Record Keeping: Maintain accurate records of client interactions and tasks across multiple systems.
- Service Planning: Use both technology and in-person methods to develop and implement service plans.
- Compliance: Ensure all records are HIPAA compliant.
- Resource Knowledge: Develop a deep understanding of community resources to support diverse client needs.
- Professional Development: Attend meetings and training sessions to continually improve your skills.
Additional Responsibilities:
- Continuity of Service: Provide essential coverage to maintain service quality.
- Goal Setting: Engage in professional goal setting to boost performance.
- Adaptability: Adjust to changes in state and federal rules.
Qualifications:
- Education: Bachelor’s degree in human behavioral science or a related field preferred.
- Experience: Relevant combinations of education and experience, or 5 years of verifiable experience, will be considered.
- Requirements: A working vehicle, valid CO driver’s license (unless active Military family), a clean Motor Vehicle Report (no major moving violations or suspensions in the last 3 years) and auto insurance coverage with bodily injury and property damage liability coverage are required.
