- Schedule: Monday – Friday, 8:00 AM – 4:30 PM
Job Overview:
As a Dietitian, you will play a vital role in improving participants’ health and quality of life by developing personalized nutrition plans, conducting assessments, and collaborating with an interdisciplinary care team. Required Qualifications:
- Bachelor’s degree in Food Science, Nutrition, and/or Dietetics
- Strong knowledge of medical terminology and the ability to complete written assessments and document participant care
- State Requirements:
- Colorado: Registration with the Commission on Dietetic Registration (CDR) as a Dietitian
Scope of Work:
Participant Care – 60%
- Develop and implement nutritional Plans of Care (POC) for participants, reassessing every six months or as necessary based on status changes.
- Evaluate participants’ access to food and make referrals for meal preparation assistance, Meals on Wheels, sack lunches, and grocery shopping support.
- Determine participants’ need for liquid nutritional supplements and order accordingly.
- Design and implement therapeutic dining programs as part of participant care plans.
- Provide nutrition education and guidance to participants, caregivers, and staff.
- Conduct home visits as needed to assess dietary needs and compliance.
- Develop care plans for high-risk participants, including medical therapy recommendations.
- Communicate with In-Home Service providers regarding participants’ food intake and dietary needs.
- Monitor and document food intake, therapeutic diets, and special dietary modifications, ensuring alignment with participants’ health needs.
- Calculate nutritional data from food intake records and provide recommendations.
- Keep detailed nutrition records in the participant’s medical chart and ensure all diet changes are properly communicated to relevant staff and caregivers.
- Actively participate in interdisciplinary care meetings to coordinate and monitor participant health plans.
Administration – 40%
- Conduct sanitation and safety inspections for kitchen areas and equipment.
- Monitor menu adequacy and provide recommendations for improvements.
- Update dietary documentation, including the sack lunch list, supplement sheet, and assisted dining guide.
- Attend care planning conferences, family meetings, and wound care meetings, reporting on participants’ nutritional statuses.
- Assist in training new Dietary Aides and Lead Dietary Aides.
- Ensure compliance with state food program regulations and industry standards.
