Lakeshore Talent is currently seeking candidates for a Case Manager role for one of their non-profit clients located in Aurora, CO. This is a direct hire position paying $18.80 – $21.90/hour with working hours Monday-Thursday, 40 hours a week.
This position will be primarily working remotely, but candidates must be in a commutable range to Aurora, CO for occasional meetings and training. Selected candidates must be able to pass a background check, have a valid CO driver's license, and a clean motor vehicle record. 


Provide comprehensive case management services within a continuum of high to low intensity services designed to improve or maintain each person’s abilities to function in the community. Assesses, monitors, and follows up with a person's ability to meet basic needs in moving toward a meaningful life. 


  • Ensures timely initiation and implementation of service development and coordination processes, Service Plans, annual reviews, and enrollments.

  • Ensures the accuracy and timely entry and distribution of written Service Plans, addendums, and other support documentation to all necessary parties, including, but not limited to providers, individuals, and family members.

  • Completes accurate case documentation (log notes), including targeted case management activities, within established timelines.

  • Assures timely completion of documentation needed for the provision of services and funding approval.

  • Analyzes, interprets, and applies government and legal regulations applicable to population(s) being served.

  • Coordinates individual’s services and supports (including enrollment offers, enrollments, and SIS referrals); implements Interdisciplinary Team meetings as appropriate to modify services. Uses judgment to decide appropriate levels of services based on individual and family priorities, budget consideration, and team recommendations, as appropriate.

  • Educates individuals and families regarding various state plan benefits, long-term care programs, support options, and [other] services.

  • Monitors individual status and satisfaction with services and makes adjustments to service plans as needed.

  • Maintains professional and ethical manner with all interactions and meets performance, quality, customer service, and coordination standards as assigned by the department management team.

  • Provides case management services and supports to temporarily assigned individuals; supports department and team coverage as needed.

  • Communicates in appropriate, effective, and respectful manner with individuals served, supervisors, other staff, agencies, and organizations; maintains individuals served confidentiality.

  • Serves as liaison for issues related to individuals in service between Case Management and agency staff, families and other agencies; negotiates positive outcomes for individuals in service.

  • Effectively and appropriately facilitates meetings.

  • Strong organization skills and ability to meet regular deadlines.

  • Excellent written and oral communications skills.

  • Demonstrate ability to work independently with minimal supervision.


  • Bachelor’s Degree in human behavioral sciences/related field OR 5 years of experience.

  • Personal vehicle in good operating condition for use during work.

  • Valid Colorado motor vehicle license.

  • Proof of motor vehicle insurance.

  • Working knowledge of the LTSS system, rules, and regulations—preferred.

  • Specific working knowledge of the intellectual and developmental disability system, rules and regulations—strongly preferred.

  • General knowledge of rules and regulations for Colorado Medicaid waivers and Colorado Department of Public Health and Environment—preferred.
