Lakeshore Talent is seeking a Physical Therapist to support a valued healthcare partner in Loveland, CO. This role is ideal for someone who is passionate about providing comprehensive care to older adults in a collaborative, team-oriented environment.

Schedule: Monday–Friday | 8:00 AM – 4:30 PM

Compensation: $41.73 – $54.28/hour (depending on experience; candidates outside this range may be considered)

Location: Loveland, CO – 10% local travel required to participant homes or nearby facilities
Position Summary
The Physical Therapist (PT) provides clinical evaluation, treatment planning, and direct physical therapy services to program participants aged 55 and older, offering care on an acute, restorative, or maintenance level. This role integrates physical therapy treatment plans into the broader Plan of Care (POC) and ensures compliance with organizational policies and state guidelines. This is a dynamic and highly collaborative role that involves both onsite and occasional in-home patient visits.
Key Responsibilities

Screen and evaluate participants for rehab needs, following APTA ethics and guidelines.

Collaborate with nursing home/assisted living staff and refer participants to OT/ST or medical clinic as needed.

Deliver and document treatment plans using modalities, therapeutic exercises, and mobility training.

Communicate clearly with team members, participants, caregivers, and families.

Provide written instructions, illustrations, and education around DME use and mobility support.

Delegate appropriate therapy responsibilities to PT Assistants or Rehab Aides per CO regulations.

Participate actively in interdisciplinary team meetings and quality management activities.

Maintain accurate documentation, inventory, and management of Durable Medical Equipment (DME).

Train non-rehab staff (CNA, Drivers) and orient contract PTs to systems and protocols.

Engage in ongoing improvement initiatives and contribute to efficient rehab operations.
Required Qualifications

Bachelor’s Degree in Physical Therapy

Completion of a CAPTE-accredited PT program

Active Colorado PT license (or eligible)

Minimum 1 year of experience working with the elderly or frail populations
Preferred Qualifications

Master’s (MSPT) or Doctorate (DPT) in Physical Therapy
-
2+ years of post-licensure PT experience

Bilingual skills are a plus
