Lakeshore Talent is currently seeking an In-Home Services Certified Nursing Assistant (CNA) to support participants in the Lakewood, CO area. This role provides high-quality, compassionate care in both center-based settings and participants’ homes, with a primary emphasis on in-home services.
Job Summary
The In-Home Services CNA provides routine, restorative, and supportive care by assisting with activities of daily living according to established care plans. This position requires professionalism, flexibility, and a commitment to participant-centered care.
Key Responsibilities
- Assist with activities of daily living (bathing, dressing, grooming, etc.).
- Provide restorative care, including ambulating, transferring, and skin/catheter care.
- Monitor and document vital signs, weights, appetite, and functional changes.
- Communicate changes in participant condition to the clinical supervisor.
- Perform essential household services and assist with meals.
- Participate in multidisciplinary care planning and attend required annual education.
- Ensure accurate and timely documentation of all care provided and incidents reported.
Position Details
- Location: Lakewood, CO
- Travel: Approximately 75% travel within a 30–40 mile radius (mileage reimbursed).
- Pay Rate: $23.00 – $27.00 per hour
Schedules Available:
- Tuesday – Saturday | 7:00 AM – 3:30 PM
- Sunday – Thursday | 1:00 PM – 9:30 PM
Requirements
- Current, valid Colorado CNA license.
- In-person CPR, BLS, and First Aid certifications (AHA or Red Cross only).
- Minimum of 1 year of experience with frail or elderly populations.
- High School Diploma, GED, or equivalent work experience.
- Reliable personal transportation, valid driver’s license, and auto insurance.
- Successful completion of medical compliance and background screenings.
