Lakeshore Talent is in search of an In-Home Services Certified Nursing Assistant (CNA) in Pueblo, Colorado. This is a direct-hire opportunity (with a $2,000 sign-on bonus!) supporting participants in both a center-based setting and in their homes, with a strong focus on compassionate, hands-on care.

75% travel required to participants’ homes within a 30–40 mile radius (mileage reimbursed)

Schedule: Tuesday through Saturday, 7:00 AM – 4:00 PM

Pay Rate: $20.67 – $24.81 per hour

Job Summary
The In-Home Services CNA provides routine personal care and assists participants with activities of daily living, restorative care, and supportive services. This role requires regular physical attendance at the care center and in participants’ homes.
Key Responsibilities

Assist with activities of daily living including bathing, dressing, grooming, toileting, eating, and mobility

Follow individualized plans of care and document services provided according to established policies and regulations

Monitor and record vital signs, weights, skin condition, appetite, and functional changes
-
Observe participants for changes in health status and report concerns promptly to the clinical supervisor
-
Assist with ambulation, transfers, positioning, catheter care, oxygen support, and other supportive services as assigned
-
Provide courteous assistance with meals and feeding when necessary
-
Perform household services essential to maintaining health in the home

Complete visit records accurately and in a timely manner

Participate in care planning with interdisciplinary teams
-
Attend required in-service and continuing education programs (minimum 12 hours annually)

Report incidents, accidents, or unusual occurrences according to policy

Accept assignments only within the scope of training and certification

Perform additional related duties as assigned
Required Qualifications
-
High School Diploma or GED (or equivalent relevant work experience)

CPR Certification, BLS Certification, First Aid Certification
-
Certifications must be from the American Heart Association or American Red Cross (in-person only)
-
Current and valid Colorado Certified Nursing Assistant (CNA) license

Reliable personal transportation with:

Valid state-issued driver’s license

Good driving record
-
Active auto insurance (DMV check required) Minimum of one (1) year of experience working with frail or elderly populations
