Lakeshore Talent is in search of an In-Home Services Registered Nurse (RN) for a direct-hire opportunity based out of Aurora, Colorado. This role focuses on delivering high-quality, skilled nursing care to individuals in their homes and in a clinical setting, supporting the health, safety, and independence of a frail and elderly population.

Schedule (Required)
- Tuesday–Friday on, Saturday–Monday off
- Then Tuesday–Friday on, Saturday off, Sunday on
Required Qualifications
- Associate’s degree in Nursing
- Current state-issued Registered Nurse (RN) license
- Current First Aid and BLS certifications (American Heart Association and/or American Red Cross) prior to hire
- Personal transportation, valid state-issued driver’s license, good driving record, and auto insurance as required by law
- Minimum of 1 year of experience working with frail or elderly populations
Preferred Qualifications
- Bachelor’s degree in Nursing
- Certification as a Gerontological Nurse
- 3 years of healthcare experience with an emphasis in geriatrics
- Bilingual skills (Spanish, Russian, Punjabi, or Hmong) are a plus
Position Summary
The In-Home Services RN provides skilled nursing care in accordance with organizational policies and all applicable regulations. This role includes direct patient care, care coordination, clinical documentation, and collaboration with interdisciplinary teams. Physical attendance at a clinical site and/or participant homes is essential to performing job duties.
Key Responsibilities
- Triage participant and staff phone calls to determine priority of care and route appropriately
- Schedule outside and specialist appointments and assist with hospital admissions as needed
- Assist in examinations, assessments, testing, and treatment of participants
- Implement physician orders and complete certification/recertification orders and discharge summaries
- Administer prescribed medications and treatments and monitor medication compliance
- Educate participants and families on medications, treatments, and health management
- Provide general and specialized nursing care in participants’ residences
- Assess and reassess participants to determine individual needs and changes in condition
- Coordinate plans of care and ensure the least restrictive environment
- Observe, document, and report participant responses to treatments and medications
- Maintain accurate, timely clinical documentation and medical records
- Provide health counseling on an individual or group basis
- Participate in care conferences, peer review, quality management, and utilization review activities
- Supervise LPNs (every 30 days) and HHAs/CNAs (every 14 days or per regulatory requirements) and provide ongoing education
- Refer participants to physical therapy, speech therapy, occupational therapy, or medical social work services as appropriate
- Ensure quality of care through periodic participant/caregiver follow-up, care plan reviews, and data trending in collaboration with leadership
- Additional duties may be assigned as needed
