Lakeshore Talent is in search of a Physical Therapist to join a healthcare team in Tampa. This is a direct hire opportunity offering competitive pay and a $5,000 sign-on bonus. The Physical Therapist will provide clinical evaluations and treatment plans for participants 55 and older, supporting rehabilitation, restorative care, and maintenance therapy while collaborating with an interdisciplinary care team.
Position Details
- Location: Tampa, FL
- Schedule: Monday – Friday
- 7:30 AM – 4:00 PM or 8:00 AM – 4:30 PM
- Travel: 10%–25% travel to participant homes and other facilities as needed (mileage reimbursed and drive time included in hours worked)
- Pay Rate: $38.80 – $50.48 per hour
- Sign-On Bonus: $5,000
Key Responsibilities
- Conduct physical therapy screenings and evaluations to determine rehabilitation needs at acute, restorative, or maintenance levels.
- Develop and implement treatment plans and integrate therapy services into each participant’s Plan of Care (POC) as directed by physician orders.
- Provide therapeutic interventions including modalities, therapeutic exercises, and functional mobility training.
- Educate participants and caregivers on therapy plans, mobility techniques, and the use and maintenance of Durable Medical Equipment (DME).
- Communicate participant needs related to mobility, positioning, and safety with care teams, caregivers, and family members.
- Collaborate with interdisciplinary team members to coordinate comprehensive care plans.
- Delegate therapy activities to Physical Therapist Assistants and rehabilitation aides when appropriate.
- Conduct home visits and provide educational sessions when indicated in the care plan.
- Maintain timely and accurate documentation of all treatments and procedures in participant medical records.
- Manage inventory and distribution of DME, including tracking loans, repairs, and returns.
- Participate in quality management initiatives, peer reviews, and interdisciplinary care meetings.
- Provide guidance and training to staff regarding participant mobility and rehabilitation needs.
Required Qualifications
- Bachelor’s Degree in Physical Therapy
- Completion of a physical therapy program accredited by the American Physical Therapy Association or another equivalent accrediting organization approved by Centers for Medicare & Medicaid Services.
- Active Physical Therapist license in the state of Florida
- Minimum 1 year of experience working with frail or elderly populations
Preferred Qualifications
- 2+ years of experience practicing as a Physical Therapist
- Master of Science in Physical Therapy (MSPT) or Doctor of Physical Therapy (DPT)
- Bilingual skills
