Case Manager – Children’s Waiver Coordination
Pay: $22.32 – $23.18/hour
Schedule: Hybrid (field + remote) in the Colorado Springs area
Position Summary
Provide case management services to children and families enrolled in Medicaid waiver programs. This role supports children with diverse needs by coordinating services and developing individualized, family-centered care plans.
Key Responsibilities
- Serve as primary contact for assigned families
- Conduct assessments and develop individualized service plans
- Coordinate care with providers, schools, and community resources
- Educate families on services, eligibility, and program updates
- Monitor services to ensure quality, safety, and compliance
- Maintain accurate documentation in electronic systems
- Conduct in-person visits in homes, schools, and community settings
Qualifications
- Bachelor’s degree or equivalent experience in human services or related field
- Experience with case management, Medicaid waivers, or working with children with disabilities preferred
- Strong communication, organization, and documentation skills
- Proficient with Microsoft Office and electronic systems
- Ability to work independently and manage multiple priorities
Requirements
- Valid driver’s license and reliable transportation
- Ability to travel locally
