Lakeshore Talent is in search of Physical Therapists for a contract opportunity supporting a dynamic interdisciplinary care team in Sacramento, California. Position Details
- Title: Physical Therapist
- Quantity: 2 openings
- Contract Length: ~3 months (temporary assignment)
- Pay Rate: $44.66 – $58.00/hour
- Location: Sacramento, CA
- Schedule: Monday – Friday, 8:00 AM – 4:30 PM
- Travel: 10%–25% (participant homes and occasional facility coverage; mileage reimbursed, drive time paid)
Position Overview
The Physical Therapist will provide comprehensive physical therapy evaluation, treatment planning, and ongoing care for program participants aged 55 and older. Care may include acute, restorative, or maintenance-level therapy based on individual needs. The PT will integrate treatment plans into each participant’s Plan of Care (POC) in alignment with physician orders and interdisciplinary team collaboration.
This role requires a strong commitment to patient-centered care, mobility support, safety education, and functional independence for older adults.
Key Responsibilities
- Conduct physical therapy screenings and evaluations for rehabilitation needs (acute, restorative, or maintenance levels)
- Develop and implement individualized treatment plans aligned with physician orders and interdisciplinary care plans
- Deliver therapeutic interventions including exercise programs, mobility training, and modality-based treatments
- Educate participants and caregivers on mobility strategies, safety techniques, and use/maintenance of durable medical equipment (DME)
- Coordinate with nursing, occupational therapy, speech therapy, and medical providers to support integrated care planning
- Perform home visits and document participant progress and caregiver feedback in medical records
- Communicate clearly with families, caregivers, and staff regarding care recommendations and participant needs
- Delegate appropriate therapy tasks to Physical Therapist Assistants (PTAs) and rehab aides as permitted by regulations
- Participate in interdisciplinary team meetings, peer review, and quality management activities
- Manage, track, and support DME inventory, loans, repairs, and replacements
- Ensure all documentation is accurate, timely, and compliant with organizational and regulatory standards
- Support onboarding and education of contract therapy staff and non-rehab personnel as needed
- Contribute to continuous improvement efforts within the rehab department
Required Qualifications
- Bachelor’s Degree in Physical Therapy
- Completion of a physical therapy program accredited by APTA or equivalent CMS-approved organization
- Active Physical Therapist license in the state of California (must be included with submission)
- Minimum of 1 year experience working with frail or elderly populations
Preferred Qualifications
- 2+ years of experience as a practicing Physical Therapist
- Master’s Degree (MSPT) or Doctorate in Physical Therapy (DPT)
- Experience in geriatric, home health, or community-based care settings
- Bilingual abilities are a plus
