Lakeshore Talent is in search of a Physical Therapist for a 12-week contract opportunity supporting a senior-focused healthcare program in Thornton, CO. Schedule: Monday – Friday, 8:00 AM – 4:30 PM

Travel: Approximately 10% – 25%
- Travel to participant homes and nearby centers as needed
- Mileage reimbursement provided
- Drive time included in hours worked
Position Overview
The Physical Therapist provides comprehensive physical therapy evaluations, treatment planning, and therapeutic interventions for adults age 55 and older. This role focuses on helping participants maintain mobility, independence, safety, and overall quality of life through individualized rehabilitation programs integrated into an interdisciplinary care plan.
This opportunity offers a lighter caseload than traditional hospital or acute care settings, with an average of up to four assessments per day and the majority of work performed onsite at the center.
Key Responsibilities
- Perform physical therapy evaluations and develop individualized treatment plans.
- Assess participants for acute, restorative, and maintenance rehabilitation needs.
- Provide therapeutic interventions including:

Therapeutic exercise

Functional mobility training

Balance and gait training

Modalities as appropriate
- Integrate therapy goals into participants’ overall Plan of Care.
- Collaborate with physicians, nurses, occupational therapists, speech therapists, and interdisciplinary team members.
- Educate participants, family members, and caregivers on treatment plans, mobility strategies, and safety recommendations.

- Make home visits as needed to assess functional mobility and provide caregiver education.
- Document evaluations, treatment plans, progress notes, and outcomes in a timely and accurate manner.
- Provide guidance and supervision to Physical Therapist Assistants and Rehabilitation Aides.
- Assess, distribute, maintain, and track Durable Medical Equipment (DME).
- Participate in interdisciplinary team meetings, quality improvement initiatives, and peer review activities.
- Provide training and education to staff regarding participant mobility, safety, positioning, and rehabilitation needs.
- Support rehabilitation services across multiple centers as needed.
Required Qualifications
- Bachelor’s Degree in Physical Therapy.
- Graduate of a physical therapy program accredited by the American Physical Therapy Association (APTA) or another CMS-approved equivalent program.
- Active Physical Therapist license in the state of California.
- Minimum one year of experience working with frail, elderly, or geriatric populations.
- Current CPR/BLS Certification through the American Heart Association or American Red Cross.
- Current First Aid Certification through the American Heart Association or American Red Cross.
Preferred Qualifications
- Two or more years of experience as a practicing Physical Therapist.
- Master of Science in Physical Therapy (MSPT) or Doctor of Physical Therapy (DPT).
- Experience working with interdisciplinary geriatric care teams.
- Bilingual skills are a plus.
