Lakeshore Talent is in search of a Certified Nursing Assistant (CNA) – In-Home Services for a contract-to-hire opportunity with a respected healthcare organization serving older adults throughout the Denver metro area. Position Details
- Employment Type: Contract-to-Hire (3-month contract to start)
- Pay Rate: $23.00 – $27.00 per hour
- Schedule: Sunday – Thursday, 9:00 AM – 5:30 PM
- Location: Based in Thornton, CO with approximately 75% local travel
- Travel Requirements: Daily travel to participant homes within a 30–40 mile radius; mileage reimbursement provided
Position Summary
The In-Home Services CNA is responsible for providing routine personal care, restorative support, and assistance with activities of daily living for elderly participants in their homes. This role works closely with nurses and interdisciplinary care teams to ensure participants receive high-quality, compassionate care while maintaining their independence and dignity.
Key Responsibilities
- Assist participants with activities of daily living, including:
- Bathing
- Dressing
- Grooming and personal hygiene
- Toileting
- Feeding and meal assistance
- Ambulation and transfers
- Monitor and document participant health status, including:
- Vital signs
- Monthly weights
- Appetite and nutritional intake
- Skin condition
- Changes in functional ability
- Follow established care plans and accurately document services provided.
- Communicate participant concerns, changes in condition, accidents, or unusual occurrences to the clinical supervisor.
- Assist with restorative and supportive care activities, including positioning, mobility assistance, catheter care, oxygen support, and other assigned tasks.
- Participate in care planning meetings and collaborate with multidisciplinary care teams.
- Complete visit documentation and records in accordance with agency policies.
- Perform basic procedures as directed by nursing or therapy staff.
- Provide household support services related to participant health and safety.
- Attend required training, continuing education, and mandatory in-service programs.
- Participate in quality improvement initiatives as requested.
Required Qualifications
- Current and valid Colorado Certified Nursing Assistant (CNA) certification.
- Current CPR certification.
- Current Basic Life Support (BLS) certification through the American Heart Association or American Red Cross.
- Current First Aid certification through the American Heart Association or American Red Cross.
- High School Diploma, GED, or equivalent work experience.
- Minimum of one year of experience working with frail, elderly, or geriatric populations.
- Valid driver's license, reliable personal vehicle, and current auto insurance meeting state requirements.
- Clean driving record.
- Ability to travel throughout the service area to participant homes.
Preferred Qualifications
- Bilingual abilities are highly preferred.
- Previous experience providing in-home care or home health services.
