Lakeshore Talent is in search of an In-Home Services Coordinator to join a healthcare team supporting the coordination and delivery of in-home services for clients in the Loveland, Colorado area. This is a great opportunity for an organized, compassionate professional with strong scheduling skills and healthcare experience who enjoys coordinating with clients, caregivers, and internal teams.
Position: In-Home Services Coordinator
Job Type: Contract – 3 months to start, with potential for conversion
Reason for Opening: Leave coverage
Pay Rate: $20.67–$24.81/hour
Location: Remote – must be able to support the Loveland, CO area
Schedule: Monday–Friday, 8:00 AM–4:30 PM
Important: Candidates must have previous EPIC experience and strong scheduling experience.
About the Role
The In-Home Services Coordinator serves as a central point of coordination between clients, families/caregivers, field staff, and the care team. This position is responsible for maintaining accurate schedules, coordinating caregiver visits, addressing scheduling changes, and ensuring clients receive the appropriate services without gaps in care.
A typical day may include:

Coordinating schedules for clients and in-home caregivers.

Managing daily and ongoing schedules for field staff and client visits.

Responding to approximately 10–15 caregiver messages per day, primarily related to scheduling changes, updates, and visit coordination.

Ensuring visits are scheduled according to care plans and provider orders.

Communicating schedule changes to clients, caregivers, and field employees in a timely and professional manner.

Reviewing reports and records to identify missing documentation or gaps in medical records.

Ensuring cancellations, completed visits, and schedule changes are properly documented.

Working extensively in EPIC, the team's primary electronic medical record system.

Maintaining confidentiality and protecting sensitive client information.

Collaborating with internal teams and field staff to resolve scheduling or service-related issues.

Participating in team meetings and daily check-ins to communicate important client and staffing updates.

Answering phones and assisting with inquiries or referrals as needed.
Administrative & Coordination Responsibilities

Maintain accurate client records and documentation in accordance with applicable requirements and organizational policies.

Use scheduling and electronic health record systems to support daily operations and reporting.

Schedule field staff in an efficient manner while considering geographic location, travel time, mileage, and overtime.

Help ensure appropriate staffing coverage based on client needs and care plans.

Communicate professionally with clients, families, caregivers, and field staff.

Assist with onboarding and orientation of new field employees as needed.

Support training and development of field staff when requested.

Assist management with operational needs and special projects.

Build strong working relationships with field staff and colleagues to promote teamwork and a positive work environment.

Attend occasional center-based meetings as needed.

Required Qualifications:

High School Diploma or GED.

At least 2 years of related administrative experience.

Previous EPIC experience – required.

Strong scheduling and coordination experience.

Experience working with electronic health records or healthcare systems.

Excellent communication and customer service skills.

Strong attention to detail and organizational skills.

Ability to work independently in a remote environment while collaborating effectively with a team.

Must be familiar with the Loveland, CO geographic area to support efficient scheduling.

Valid driver's license, reliable personal transportation, good driving record, and auto insurance as required by law.

Current First Aid and BLS certifications, or ability to obtain prior to hire. Certifications must be through the American Heart Association or American Red Cross.
Preferred Qualifications:

2+ years of experience working with seniors, gerontology, medical services, or social services.

At least 1 year of scheduling or logistics management experience.

At least 1 year of healthcare experience.

Familiarity with the physical and emotional needs of seniors and individuals who may have physical or cognitive impairments.
