Job Title: CMA Coordinator
Pay: $22.32 – $23.69/hour
Schedule: Hybrid (field + remote) in the Colorado Springs area
Position Summary
The CMA Coordinator supports individuals in accessing and coordinating long-term care services. This role works closely with individuals, families, and providers to develop person-centered service plans that promote independence, health, and safety.
Key Responsibilities
- Develop and coordinate individualized service plans
- Serve as a primary contact for individuals, families, and providers
- Monitor services to ensure quality, compliance, and effectiveness
- Advocate for individuals and connect them to community resources
- Maintain accurate documentation in electronic systems
- Conduct in-person visits in homes, facilities, and community settings
- Participate in team meetings, trainings, and ongoing development
Qualifications
- Bachelor’s degree or 5 years of relevant experience in human services, case management, or working with vulnerable populations
- Experience with Medicaid waivers or long-term care services preferred
- Strong communication, organization, and problem-solving skills
- Comfortable with technology, databases, and Microsoft 365
Requirements
- Valid driver’s license and reliable transportation
- Clean Motor Vehicle Report, no major moving violations in the past 3 years
- Ability to travel locally
