Job Title: CMA Coordinator 

Pay: $22.32 – $23.69/hour
Schedule: Hybrid (field + remote) in the Colorado Springs area


Position Summary

The CMA Coordinator supports individuals in accessing and coordinating long-term care services. This role works closely with individuals, families, and providers to develop person-centered service plans that promote independence, health, and safety.


Key Responsibilities

  • Develop and coordinate individualized service plans
  • Serve as a primary contact for individuals, families, and providers
  • Monitor services to ensure quality, compliance, and effectiveness
  • Advocate for individuals and connect them to community resources
  • Maintain accurate documentation in electronic systems
  • Conduct in-person visits in homes, facilities, and community settings
  • Participate in team meetings, trainings, and ongoing development

Qualifications

  • Bachelor’s degree or 5 years of relevant experience in human services, case management, or working with vulnerable populations
  • Experience with Medicaid waivers or long-term care services preferred
  • Strong communication, organization, and problem-solving skills
  • Comfortable with technology, databases, and Microsoft 365

Requirements

  • Valid driver’s license and reliable transportation
  • Clean Motor Vehicle Report, no major moving violations in the past 3 years
  • Ability to travel locally
