Lakeshore Talent is in search of an Occupational Therapist (OT) for a contract opportunity in Philadelphia, Pennsylvania. This is a 3-month contract role with competitive pay, a consistent weekday schedule, and the opportunity to make a meaningful impact serving an elderly population.


Position Details

  • Job Title: Occupational Therapist (OT)

  • Contract Length: 3 months

  • Location: Philadelphia, PA (onsite clinical setting)

  • Travel: 10%–25% travel to participants’ homes and to support other facilities as needed

    • Mileage reimbursed

    • Drive time included in hours worked

  • Pay Rate: $40.00–$52.00 per hour

  • Schedule: Monday–Friday, 8:00 AM–4:30 PM (30-minute lunch)
     


Required Qualifications

  • Graduate of an Occupational Therapy curriculum accredited by the American Occupational Therapy Association (AOTA)

  • Degree in Occupational Therapy:

    • Bachelor’s, Master’s (MOT), or Doctorate (OTD)

  • National certification by AOTA or eligibility to sit for the National Registration Examination

  • Active state OT license in the state of practice

  • Valid state-issued driver’s license, reliable personal transportation, good driving record, and required auto insurance

  • Minimum of one (1) year of experience working with frail or elderly populations


Preferred Qualifications

  • Two (2) years of experience preferred

  • At least one (1) year of general occupational therapy experience in a geriatric setting


Job Responsibilities

  • Provide comprehensive occupational therapy evaluations and develop individualized treatment plans

  • Deliver acute, restorative, and maintenance-level occupational therapy services

  • Assess participant conditions and collaborate with participants and caregivers on therapy implementation

  • Integrate occupational therapy plans into the interdisciplinary Plan of Care

  • Administer therapy services based on referrals and interdisciplinary assessments

  • Educate participants, families, caregivers, and staff on safety, ADLs, exercise programs, and use of durable medical equipment (DME)

  • Assist with ordering, inventory, distribution, and maintenance of DME

  • Conduct home visits and educational sessions as indicated and document outcomes in the medical record

  • Monitor and document participant progress, maintaining timely and accurate clinical documentation

  • Prepare evaluations, progress notes, discharge summaries, and physician reports

  • Participate in care planning meetings, staff meetings, patient care conferences, and interdisciplinary team activities

  • Support quality assurance, utilization review, peer review, and performance improvement initiatives

  • Maintain therapy equipment and ensure a clean, safe, and organized therapy environment

  • Adhere to all organizational policies, safety standards, and regulatory requirements

  • Perform additional duties as assigned


Onboarding Requirements

  • Proof of degree completion and accredited OT curriculum

  • Active OT certification/licensure in the state of practice

  • Valid driver’s license, DMV check, and proof of insurance

  • Background check and drug screen

  • BLS, First Aid, and CPR certifications

  • Physical exam / medical clearance

  • Flu vaccination (or signed declination with mask compliance)
