Lakeshore Talent is in search of a Care Plan Coordinator to support a healthcare center in San Bernardino. This is a contract or contract-to-hire opportunity offering the chance to work closely with an interdisciplinary care team to coordinate patient care plans, ensure regulatory compliance, and support high-quality healthcare delivery.

Position Details

  • Contract Length: 6 months to start with potential to extend or move perm!
  • Pay Rate: $23.00 – $26.11 per hour
  • Schedule: Monday – Friday | 8:00 AM – 5:00 PM PST
  • Location: San Bernardino, CA

Position Overview

The Care Plan Coordinator is responsible for overseeing and coordinating the development, documentation, and completion of participant care plans in accordance with federal and state regulations. This role works closely with interdisciplinary healthcare teams to ensure assessments and care plans are completed accurately, on time, and in alignment with clinical best practices. The coordinator plays a key role in facilitating care planning discussions, monitoring compliance, and ensuring documentation supports the highest possible level of participant care.

Key Responsibilities

  • Oversee and coordinate the development and completion of participant care plans in compliance with federal and state regulations.
  • Track completed care plans for compliance and maintain the Care Plan schedule spreadsheet.
  • Facilitate interdisciplinary care plan discussions and engage the team to ensure appropriate documentation and alignment between diagnoses, treatment plans, and services.
  • Audit EMR documentation and collaborate with team members to ensure assessments and care plans are accurate and complete.
  • Monitor the assessment and care planning process to ensure timeliness and regulatory compliance.
  • Participate in center committees related to Quality Assessment and Assurance as assigned by leadership.
  • Work collaboratively with interdisciplinary teams to support timely and accurate assessments and care planning documentation.
  • Coordinate scheduling, monitoring, and auditing of care plan documentation.
  • Develop and provide training on care plan processes for new hires and annual refresher training for staff.
  • Promote positive relationships with participants, families, and regulatory agencies.
  • Maintain a professional presence and ensure adherence to organizational compliance, conduct, and business ethics policies.
  • Participate in staff meetings, educational activities, and in-service trainings.
  • Contribute to quality improvement initiatives as assigned.

Required Qualifications

Candidates must possess one of the following credentials:

  • Active LPN license
  • Graduate of an accredited Medical Assistant program
  • Current Certified Nursing Assistant (CNA) license
  • Bachelor’s degree in a healthcare-related field

Additional requirements include:

  • Current CPR certification or ability to obtain certification
  • 3+ years of experience in a healthcare setting
  • Experience with EMR systems (EPIC or similar)
  • Proficiency with Microsoft Office, including Excel
  • Experience with healthcare assessments, scheduling systems, and care plan software
  • Strong written and verbal communication skills
  • Strong analytical and decision-making abilities
  • Knowledge of healthcare regulations, nursing practices, and patient care procedures
  • Ability to maintain consistent attendance and work collaboratively within a team environment

 

Pre-Employment Requirements

Candidates selected for the role must complete standard healthcare onboarding requirements, including but not limited to:

  • Relevant clinical certification verification (CNA, MA, LPN, etc.)
  • CPR/BLS Certification (American Red Cross or American Heart Association – in-person)
  • First Aid Certification (American Red Cross or American Heart Association – in-person)
  • Physical exam and medical clearance
  • TB test (within 1 year)
  • Seasonal flu vaccination (October–April) or flu declination with masking requirement
  • Background check 
  • Drug screening 
  • Completion of compliance and healthcare training documentation including HIPAA, emergency preparedness, infection control, and related policies
