Lakeshore Talent is in search of an In-Home Services Coordinator for a Direct Hire opportunity at a well-established healthcare organization located in Aurora, Colorado. This is an excellent role for someone with strong scheduling experience who is passionate about supporting older adults and coordinating essential in-home services.

Location:  Aurora, CO 
Schedule: Tuesday – Saturday (Saturday is remote)
Pay Range: $19.23–$24.81/hour


Overview:

The In-Home Services Coordinator is responsible for ensuring the smooth and efficient delivery of services to individuals receiving in-home care. This includes managing detailed care schedules, overseeing field staff logistics, and ensuring services align with individualized plans and provider directives. This role offers a balance of administrative duties and direct coordination with clients and caregivers.


Key Responsibilities:

Client Support & Scheduling (50%)

  • Coordinate daily and ongoing in-home care schedules for field staff, ensuring alignment with care plans and provider orders.

  • Serve as a liaison between clients, families, and caregivers, providing excellent communication and service.

  • Notify all parties of schedule changes promptly and document changes in the system.

  • Conduct occasional supervisory home visits to ensure services are meeting quality standards.

  • Participate in daily team huddles and on-call rotation as needed.

  • Maintain real-time updates to client schedules and verify completion of scheduled tasks.

Administrative Support & Compliance (50%)

  • Maintain accurate client documentation in accordance with federal, state, and local guidelines.

  • Use internal software systems to track compliance and operational performance.

  • Support onboarding and orientation of new care staff and assist in training and performance management.

  • Ensure efficient staff scheduling to minimize overtime and travel time.

  • Provide administrative support to leadership and assist with backup duties as assigned.

  • Participate in internal meetings and contribute to a positive and team-oriented workplace culture.


Qualifications:

  • High School Diploma or GED

  • Valid driver’s license, personal vehicle, auto insurance, and a clean driving record

  • 2+ years of experience in gerontology and/or medical or social services

  • 1+ year of scheduling or logistics coordination experience

  • 1+ year in a healthcare-related role

  • Strong knowledge of the Aurora/Denver metro area for efficient scheduling

  • Current First Aid and BLS certifications (American Heart Association or Red Cross), or ability to obtain prior to start
