Lakeshore Talent is in search of an In-Home Services CNA in Philadelphia, Pennsylvania for a contract opportunity supporting a PACE (Program of All-Inclusive Care for the Elderly) center. This position is a 3-month contract to start, with potential for extension and/or conversion to a permanent role.
This is a rewarding opportunity to provide hands-on care to frail and elderly participants both at the PACE center and in their homes, helping them maintain independence and quality of life.
Position Details
- Location: Philadelphia, PA
- Travel: Approximately 75% travel to participants’ homes within a 30–40 mile radius (mileage reimbursed)
- Schedule: Monday–Friday, 7:00 am–3:30 pm + every other weekend rotation
- Contract Length: 3 months to start, with possible extension and/or conversion
- Pay Rate: $20.67 – $24.81 per hour
Position Overview
The In-Home Services CNA is responsible for providing routine, restorative, and supportive care to elderly participants in both center-based and home settings. This role focuses on assisting with activities of daily living (ADLs), monitoring participant health status, and supporting interdisciplinary care planning.
Key Responsibilities
- Follow individualized care plans and assist with ADLs including bathing, dressing, grooming, toileting, feeding, and mobility.
- Provide restorative and supportive care such as ambulation, positioning, transferring, skin care, catheter care, and oxygen assistance.
- Monitor and document vital signs, weights (monthly), appetite, skin condition, and changes in functional ability.
- Accurately document care provided in accordance with policies and regulatory requirements.
- Report health changes, variances, accidents, or unusual occurrences to clinical supervisors promptly.
- Assist with household services essential to healthcare in the home.
- Participate in care planning with interdisciplinary teams.
- Attend required in-service trainings (minimum 12 hours annually) and quality management activities.
- Complete visit records and documentation in a timely manner.
- Perform simple procedures as delegated by nursing or therapy staff.
- Provide care only within scope of training and licensure.
Required Qualifications
- Current and valid Pennsylvania CNA license.
- Current CPR, BLS, and First Aid certifications (in-person only; American Heart Association or American Red Cross).
- Minimum one year of experience working with the frail or elderly population.
- High School Diploma or GED (or relevant experience in lieu of education).
- Reliable personal transportation.
- Valid driver’s license, good driving record, and current auto insurance (proof required; DMV check conducted).
- Willingness to travel to participant homes.

Onboarding Requirements
- CNA certification verification.
- CPR/BLS and First Aid certifications (in-person).
- Physical exam within 30 days and medical clearance form signed by physician.
- TB test (within 1 year).
- Seasonal flu vaccination (October–April) or signed flu declination (mask required if declining).
- Proof of car insurance and valid driver’s license.
- Background check (including OIG) and Motor Vehicle Report (MVR).
- Drug screening (9-panel and 4-panel required for clinical roles).
- Completion of required training acknowledgements and compliance documentation.
