Pay: $44.66 – $58.08 per hour | Salary Range: $93,000 – $120,800 (DOE)
Travel: Up to 25% of the time to participant homes within a 70-mile radius (Los Lunas to Bernardino County areas)


Key Responsibilities:

  • Perform home safety evaluations for participants moving into new living environments.
  • Assess and order durable medical equipment (DME) needs, such as grab bars and ramps.
  • Provide clinical occupational therapy evaluations and treatment plans, working within an interdisciplinary team.
  • Conduct annual participant assessments and respond to PCP referrals.
  • Educate participants, caregivers, and staff on maximizing safety and ADL functional abilities.
  • Document all occupational therapy services and maintain accurate department record-keeping.
  • Participate in interdisciplinary team meetings and contribute to individualized participant care plans.
  • Travel to participant homes 2-3 times per week; mileage is reimbursed.

Qualifications:

Required:

  • Bachelor’s, Master’s (MOT), or Doctorate (OTD) in Occupational Therapy from an accredited program.
  • National Certification by the American Occupational Therapy Association or eligibility to sit for the National Registration Exam.
  • Licensed and registered in the state of California.
  • Valid driver’s license, personal vehicle, clean driving record, and auto insurance.
  • Minimum of 1 year experience; geriatrics/elderly care experience highly preferred.
  • Open to new graduates who are eager to learn.

Preferred:

  • 2+ years of experience in occupational therapy, ideally in a geriatric setting.

Skills and Software:

  • Strong organizational and time management skills.
  • Comfortable working independently in both clinical and home settings.

Proficiency with:

  • EPIC EMR
  • Outlook
  • Microsoft Teams
