SIGN ON BONUS: $10,000
Salary Range: $110,600 – $143,800 annually
Location: San Bernardino, CA 
Schedule: Monday – Friday | 8:00 AM – 5:00 PM (On-site)
Start Date: Immediate


About the Role:

As the RN Manager – Home Care, you will be at the heart of ensuring top-quality care for participants in a home environment. You will lead and inspire a multidisciplinary team including RNs, Schedulers, Medical Records staff, and CNAs — with field-based clinical staff and office-based operations staff under your guidance. This is an exciting leadership opportunity for a motivated RN who thrives on collaboration, critical thinking, and operational excellence.

You’ll also be a vital contributor to the Interdisciplinary Team (IDT), working closely with colleagues to coordinate personalized care plans that honor the dignity and independence of each participant.


Key Responsibilities:

  • Leadership and Supervision (90%): Manage hiring, training, coaching, performance appraisals, and team development. Oversee scheduling, expenditures, and ensure outstanding staff morale and professionalism.

  • Participant Care (10%): Conduct initial and ongoing participant assessments, develop and coordinate care plans, provide nursing interventions, medication administration, and annual reassessments.

  • Actively participate in IDT meetings and ensure field visits to observe and support staff throughout their tenure.

  • Utilize Epic software for medical records and care coordination.


What You Bring:

  • Current RN license in the state of California (required).

  • Bachelor of Science in Nursing (BSN) required.

  • 2+ years RN healthcare experience with a focus on geriatrics.

  • 1+ year of direct supervisory or administrative experience.

  • Strong leadership skills with high emotional intelligence and critical thinking abilities.

  • Valid driver’s license, transportation, and auto insurance.

  • Current First Aid and BLS certification (American Heart Association or Red Cross accepted).

  • Expertise in home health, primary care, or health facility environments within the past five years.
