Lakeshore Talent is partnering with a well known Non-Profit out of Colorado Springs in their search for a Case Manager Supervisor to support  provides leadership and operational oversight for a team of service coordinators.

This role ensures that case management practices meet all applicable state regulations, program requirements, and person-centered standards. The supervisor is responsible for monitoring performance, coaching staff, and supporting process improvements to ensure timely, accurate, and high-quality services.

Pay: $63,500 – $66,500/year, depending on experience

Key Responsibilities

Regulatory and Program Compliance

  • Maintain expert knowledge of state Medicaid LTSS rules, waiver-specific regulations, and program updates.
  • Ensure assigned team members meet requirements for assessments, service plans, incident tracking, and program transitions.

Supervision & Coaching

  • Oversee a team of up to 15 coordinators, providing regular feedback, performance evaluations, and development plans.
  • Conduct individual supervision meetings and team check-ins to promote performance excellence and retention.

Quality Assurance

  • Review case documentation and data entries to ensure compliance and accuracy.
  • Provide training and guidance when documentation or processes require improvement.

Training & Professional Development

  • Support new hire onboarding and ongoing staff training on updated policies, systems, and best practices.
  • Encourage staff to participate in required trainings and professional growth opportunities.

Process Improvement & Redesign Initiatives

  • Lead efforts to standardize workflows across waiver types.
  • Ensure timelines are met for assessments, service planning, and initial contacts.

Collaboration & Representation

  • Act as a liaison between internal leadership, state agencies, and partner organizations.
  • Address escalated concerns and participate in collaborative workgroups or corrective action plans when needed.

Team Culture & Engagement

  • Promote a supportive, inclusive, and trauma-informed team environment.
  • Encourage wellness, collaboration, and shared accountability.

Data Tracking & Reporting

  • Monitor and report on key performance indicators such as timeliness, documentation accuracy, and audit results.
  • Use data to inform coaching and improvement strategies.

Additional Duties

  • Provide coverage during staff vacancies.
  • Assist in hiring and interviewing new team members.
  • Adapt to policy or program changes that affect operations.

Qualifications

Required

  • Bachelor’s degree in a human services-related field OR significant experience working with Medicaid LTSS programs.
  • Experience working with diverse populations and understanding person-centered planning principles.

Preferred

  • Two or more years in case management roles within Medicaid waiver programs.
  • At least one year of supervisory or team lead experience in health care, behavioral health, or social services.
  • Familiarity with state case management systems and process redesign initiatives.

Skills & Knowledge

  • Strong organizational and time management skills.
  • Ability to interpret and apply complex regulations.
  • Proficiency in Microsoft 365, virtual communication tools, and secure documentation systems.

Work Environment:
This role involves both office and community-based work, occasional home visits, and virtual meetings. Moderate physical activity such as walking, sitting, and light lifting may be required. Reliable transportation, valid driver’s license, and required insurance coverage are necessary.
