Lakeshore Talent is in search of an experienced and compassionate RN Manager – Home Care to join an industry-leading organization making a meaningful difference in the lives of seniors.

 The RN Manager – Home Care plays a critical leadership role in ensuring the delivery of high-quality, coordinated care for participants in their home environments. This position offers a unique chance to lead a team of healthcare professionals in a collaborative, mission-driven setting while making a tangible impact on the daily lives of those served.


Position Details

  • Salary: $96,100-$125,000/yr

  • Location: Salem, VA

  • Schedule: Monday – Friday | 8:00 AM – 5:00 PM | On-Site

The RN Manager – Home Care is responsible for managing both clinical and operational staff, including RNs, CNAs, schedulers, and medical records personnel. The role focuses on ensuring quality home care services, effective staff supervision, and regulatory compliance. The ideal candidate is a licensed RN with strong leadership skills, high emotional intelligence, and experience in home health, hospice, or long-term care environments.

This leader will be a key member of the Interdisciplinary Team (IDT), contributing to care planning and coordination. While there are no formal on-call requirements, flexibility and commitment are essential.


Key Responsibilities

Supervision & Administrative (90%)

  • Lead and support field-based clinical staff and on-site operational teams.

  • Manage hiring, training, scheduling, and performance evaluations.

  • Monitor staff workload, overtime, and resource allocation.

  • Promote a positive team culture emphasizing collaboration, quality, and accountability.

  • Conduct regular one-on-ones and team meetings to align goals and improve communication.

  • Oversee home care quality through periodic reviews and participant/caregiver feedback.

  • Participate in IDT meetings to contribute to interdisciplinary care strategies.

Participant Care (10%)

  • Perform in-home assessments to develop individualized plans of care (POC).

  • Coordinate personal care services and ensure ongoing needs assessments.

  • Deliver skilled nursing care as needed and monitor participant progress.

  • Administer and educate on medication compliance.

  • Maintain accurate, up-to-date participant medical records in Epic.


Required Qualifications

  • Current Registered Nurse (RN) license in the state of Virginia.

  • Bachelor of Science in Nursing (BSN) required.

  • 2+ years of RN experience, with an emphasis on geriatrics.

  • 2+ years of leadership experience, including staff supervision and scheduling.

  • Valid driver’s license, reliable transportation, and auto insurance as required by law.

  • Current First Aid and BLS certifications (American Heart Association or Red Cross).

  • Proficient in Epic or similar electronic health record systems.


Preferred Background

  • Experience in Home Health, Home Care, Hospice, SNF, or LTC settings.

  • Proven ability to thrive in complex, matrixed environments.

  • Strong communication, problem-solving, and collaboration skills.


Interview Process

  • 2–3 rounds, including:

  • Virtual interview(s)

  • On-site visit at the Salem, VA center
