Lakeshore Talent is in search of an In-Home Services Coordinator for a direct hire opportunity at a respected healthcare organization in Charlottesville, VA.


Position Overview:

  • Location: Charlottesville, VA 
  • Schedule: Monday through Friday, 8:00 AM – 4:30 PM
  • Pay Rate: $19.23–$23.08/hr 

Must-Have Qualifications:

  • High School Diploma or GED
  • Valid state-issued driver’s license, personal transportation, good driving record, and auto insurance as required by law
  • Current First Aid and BLS certifications (American Heart Association or Red Cross only)
  • At least 2 years of experience in gerontology and/or medical/social services
  • Minimum 1 year of experience in scheduling or logistics management
  • Minimum 1 year in the healthcare field
  • Strong knowledge of the local geographic area for effective scheduling
  • Understanding of the physical and emotional needs of seniors with physical/mental impairments

Key Responsibilities:

Client Support & Oversight (50%)

  • Serve as liaison between clients, families, caretakers, and the healthcare organization’s In-Home Services team
  • Ensure schedules align with Plan of Care (PoC) and provider orders
  • Coordinate services with other community agencies or internal departments
  • Document and communicate schedule changes to clients and staff
  • Conduct routine and unscheduled supervisory visits to client homes
  • Participate in daily team stand-ups and on-call rotation
  • Maintain up-to-date records of home visits and service delivery tasks
  • Address and resolve service-related issues or complaints in collaboration with leadership
  • Participate in the selection and oversight of care workers to ensure quality care

Administrative Operations (50%)

  • Maintain client records in compliance with federal, state, and organizational standards
  • Utilize software systems for scheduling and operational reporting
  • Assist with orientation, training, and development of Personal Care Workers (PCWs)
  • Provide performance feedback and lead in-service training sessions as needed
  • Fill in to perform PCW duties when necessary
  • Schedule staff in the most efficient and cost-effective manner
  • Ensure accurate and timely communication across electronic health records
  • Answer and transfer incoming calls professionally
  • Accept intake and referral calls, representing the organization positively
  • Support operational goals including quality, census, and revenue targets
  • Collaborate with leadership on standard procedures and backup management duties
  • Foster positive morale and teamwork across staff and departments
  • Attend internal meetings as needed to support coordination and care planning
